New GoutPatient

GoutHelper starts with baseline information about the patient, his or her gout, and its related treatments.

IMPORTANT: All GoutPatients are assumed to have a history of gout. If the patient doesn't have a history of gout and it's unclear if the symptoms are due to gout, create a Flare instead. This can always be converted into a GoutPatient.
